Use the online scheduler to view available appointments and to book a session.  If you would prefer to book an appointment over the phone, you can call Monica at (360) 930-9052.

If this is your first session, please arrive 10 minutes early to fill out a necessary intake form and discuss your treatment needs with Monica. For your convenience, you can download a copy of the intake form if you would like to complete it in advance.

During the session, you may undress to your comfort level. You may choose to leave on as much clothing as needed for your comfort or remove all articles of clothing, including undergarments, as you will always be draped (covered with a sheet) and only the area being worked on will be uncovered. 


Insurance

I am a preferred provider with First Choice Health Insurance and Shasta Administrative Services.  If you are insured by either company, please bring your insurance card and medical referral (if applicable) to your appointment so that I am able to bill your insurance.  If you are not eligible for massage therapy through your insurance, you are responsible for the cost of services.

Unfortunately, insurance companies in Washington State are not currently adding massage therapists to their networks so I am unable to bill other insurance company for services.  However, some insurance companies allow for individuals to receive services from an out of network provider and receive reimbursement for some or all of the cost. Please check with your insurance company as to whether or not this is an option for you as I am more than willing to provide you with the necessary documentation to submit for reimbursement.  

Payment

Payment is due at the time of service and Olympic Peninsula Massage accepts credit cards (Visa, MasterCard, American Express, Discover, JCB, or UnionPay), Paypal, cash or check.  If writing a check, please make the payment out to Monica Henry.  If paying with cash, please bring the exact amount.

New clients must complete an Intake/Health History Form and review the Olympic Peninsula Massage Client Policies Form.  If you  have access to a printer, you can complete these in advance and bring them with you to your appointment.  This will allow for more time during the session to address your massage therapy needs.

Olympic Peninsula Massage Client Policies

  
It is my intention to provide clients with professional and therapeutic massage services and to create an environment that promotes well-being. The following policies serve as a guide for first-time and regular clients of Olympic Peninsula Massage to promote clear expectations and a positive experience.
  
  
Cancellation, Late Arrival & No Show Policy
I understand that unanticipated events occur in everyone’s life and, keeping that in mind, Olympic Peninsula Massage clients will not be charged a fee the first time a client does not provide 24 hours’ notice of a need to cancel or reschedule an appointment, are significantly late or if a client “no- shows” for an appointment*.   Subsequent cancellations/late arrivals and “no-shows” are subject to the following:
  • Cancellations: Please provide at least 24 hours’ notice if you need to reschedule or cancel an appointment.  If a client does not cancel an appointment 24 hours in advance, it will be considered a “no show” appointment and 50 percent of the scheduled service fee will be charged. Clients will not be rescheduled until the fee has been paid.
  • Late Arrivals: If a late arrival is inevitable (1 to 14 minutes), the service will be shortened in order to keep on schedule and the full-service fee will be charged. No full or partial refunds will be given.   After 15 minutes, the appointment will be considered a "no show" and 50 percent of the scheduled service will be charged.  Clients will not be rescheduled until the fee has been paid.
  • No Shows: Clients will be charged 50 percent of the fee for the scheduled service and will not be rescheduled until the fee has been paid.  Clients will not be rescheduled after 3 “no shows”
  • Therapist Cancellation:  If I must cancel an appointment due to illness or an emergency situation, at least 12 hours’ notice will be provided. If lesser notice is provided, the client will receive a 25% price reduction on their next massage.
*Illness and emergencies will be evaluated on a case-by-case basis. If you need to cancel an appointment due to illness or emergency, please call to avoid being charged a no-show fee.

Client Contact & Confidentiality
  • The discussion between the massage therapist and the client is confidential. The client may or may not choose to talk during the massage.  While I am happy to listen to your conversation, it is inappropriate for a massage therapist to discuss the therapist’s personal life with a client.
  • Client records are stored in a locked file cabinet.  Any information disclosed, observed or treated will be kept confidential and will not be disclosed to any individuals or entities without written client consent.  Exceptions: I may be required to release records under a court order and I am obligated to report suspicion of child abuse to the proper authorities.
  • In order to respect client privacy, I will only contact a client utilizing the approved method requested by the client on the client intake form. 
  • In order to protect client confidentiality, I will not approach or acknowledge a client in a public setting.


Prices, Payment & Gift Certificates

  • Payment is expected at the end of your appointment and may be made in the form of cash, check, PayPal or credit card. Olympic Peninsula Massage accepts Visa, Mastercard, Discover, American Express, JCP and Union Pay
  • Any returned checks for insufficient funds will incur a $33 processing fee
  • No refund will be made for services provided.  In lieu of a refund, a discount off subsequent sessions may be offered.
  • Gift Certificates: Gift certificates are not refundable or redeemable for cash but they are transferable.  The dollar value of a gift certificate does not expire.  However, if your gift certificate was given by Olympic Peninsula Massage as a promotional gift, it is valid until the expiration date noted on the certificate.  All gift certificates and gift cards must be presented at time of service or you must pay by other means.

New Clients, Treatment Plans & Medical Conditions
  • New Clients: If you are a new client, you will need to complete a client intake form prior to receiving massage services.   If possible, please complete this form prior to your appointment and bring it with you.  Please arrive 10 minutes before your scheduled appointment time to go over the paperwork.  The information obtained in the intake form and during intake discussion will help with the development of a treatment plan.  Additionally, certain medical conditions and medications impact the types of massage that can be provided.  If you do not arrive early enough to fill out/go over the paperwork, the time it takes will be taken out of your massage.
  • Treatment Provided: Prior to each session, the treatment that you are to receive will be discussed and only treatment that is within my scope of practice and that you agree to receive will be provided. I will "check in" with the client during the massage as needed concerning client comfort and treatment.  Clients may refuse any massage methods or stop the massage at any time.
  • Referrals: I will only use techniques within my scope of practice.  If your treatment requires techniques that are beyond my scope of practice, I will refer you to a qualified massage therapist or other health care provider.
  • Medical Conditions: Due to certain contraindications and cautions for massage, the practitioner must be made aware of existing physical and mental conditions. By signing below, I acknowledge that: I have informed the massage therapist of all my known physical and medical conditions and medications. I understand that it is my responsibility to keep the massage therapist updated on any changes in my health status and agree to do so each time I receive massage.
  • Scope of Practice: I understand that massage therapists are not licensed to practice medicine and therefore do not diagnose illness or disease; perform spinal manipulations, or prescribe medical treatments. I am aware that massage therapy is not a substitute for medical treatment, medication or diagnosis for any health condition I may have. It is recommended that I pursue appropriate health management for conditions I may have.


Consideration for Clients Who Are Minors
All children under the age of 18 are classified as minors and must have a parent/guardian signature on their Intake Form/Health History and Client Policies Form.   All children under the age of 15 must have a parent/guardian present during their session.

Clothing and Draping
Clients may undress to their comfort level. The client may choose to leave on as much clothing as needed for comfort or remove all articles of clothing, including undergarments, as the client will always be draped and only the area being worked on will be uncovered. Requests for no draping will not be accommodated in accordance with our ethical guidelines and Washington State law.

Inappropriate Sexual Behavior 
Massage therapy is strictly therapeutic and therefore strictly non-sexual.  Sexual interaction or discussion of any kind between the client and a massage therapist is never appropriate. Requests for sexual activity will not be tolerated, treatment will be immediately stopped, the full price of the scheduled service will be charged and the client will not be rescheduled.  Involuntary sensual response to massage is natural and this is not considered inappropriate sexual activity or inappropriate behavior.   

Massage Party Cancellation Policy
· A 75% refund will be made if the massage package is cancelled more than 7 calendar days before the event
· A 50% refund will be made if the massage package is cancelled 7 to 4 calendar days before the event
· A 25% refund will be made if the massage package is cancelled 3 calendar days before the event
· No refund will be made for cancellation made less than 3 calendar days before the event
· No refund will be given for missed or skipped appointments.
